Integra Codman Hakim Programmable Valve

The Codman Hakim Programmable Valve is an implantable shunt valve that drains cerebrospinal fluid to treat hydrocephalus, implanted by neurosurgeons and adjusted in clinic without further surgery. A ruby ball and cone seat held by a calibrated steel spring sets the opening pressure, and an external programmer turns a stepper motor inside the valve with a coded magnetic signal to choose one of 18 settings from 30 to 200 mmH2O.

Safety

Safety warnings from the record.

  • Strong magnetic fields, including MRI and common magnets over 80 gauss, may change the valve setting. Source
  • Valves are supplied without a programmed pressure and must be set before implantation. Source
  • Over-drainage can cause subdural haematoma and slit ventricles; postural headache relieved by lying down may be a sign. Source
  • Contraindicated in patients on anticoagulants or with a bleeding diathesis, and implantation should be delayed if infection is present. Source
  • Handle silicone parts with shod forceps and keep them off linty surfaces; cuts can rupture components and silicone attracts particles. Source

Not covered: The IFU used is the lumbo-peritoneal (L-P) version, so pressure-flow tolerances may differ for ventricular configurations; the Codman programmer and VPV manuals with their on-screen messages were not reviewed, so no error codes are given.
